Sports Physicals –  2011/12

Below are a couple of options for sports physicals for the upcoming school year.  High school physicals are required yearly.  Middle school physicals, if taken in 7th grade, are good for 2 years.  A physical form must be completed and signed by a parent/guardian and the student (see attached).   Please ask students to wear appropriate clothing.

 

                                 OPTION 1

DATES:                 May 10th & May 11th, 2011 (Tuesday & Wednesday)

                           August 2nd, 3rd & 4th  (Tuesday – Thursday)

 

LOCATION:           Billings Clinic

Orthopedic and Sports Medicine Center

2702 8th Ave. North

 

COST:                           $20.00 (payable to Billings Clinic)

 

TIMES:                 3:30 PM – 4:30 PM

 

Students are advised to call 238-5254 to schedule.  This helps to maintain a manageable number of sports physicals at the stated times.  This time should not be considered an appointment.

 

 

                                    OPTION 2

DATE:                  June 4, 2011 (Saturday)

 

LOCATION:          Ortho Montana Sports Medicine Center

                                    Yellowstone Medical Center, East Building

                                    2900 12th Ave. North, Suite 100E

 

COST:                 $20.00 (payable to the Billings Public Schools)

                                    Cash or check only

 

TIME:                           8:00 AM  -  11:00 AM

 

No appointments will be taken.  It will be first come, first serve.

 

If there is a family history of premature sudden death (age 40), a personal history of fainting, passing out, chest discomfort/shortness of breath with exercise and/or any complex medical condition with ongoing treatment, we will not be able to clear them with this sports physical.  It is suggested that these students follow-up with their family physician or pediatrician.
